Dr. Miller's Approach to Dermal Fillers

To value how facial fillers rejuvenate the face, a little knowledge of the facial aging process is helpful. The face begins to show the signs of aging most notably by the loss of facial volume. This loss of volume in the face is similar to a balloon that loses air. That is, as air or volume is lost, the balloon becomes lax, sags, and wrinkles. Unfortunately, a similar process occurs in our faces.

I often educate my patients that the loss of volume is occurring deep in the skin, affecting facial fatty tissue, collagen, and even the thickness and width of the bones in our face. Facial volume truly does give support and structure to the skin, thereby keeping the skin firm around the cheeks, eyes, jawline, and mouth. For example, the loss of volume in our temples causes our eyebrows to drop and creates an aged and hollowed appearance to our upper face. This is why maintaining or restoring volume to our face is so important.

Most current fillers such as the JUVÉDERM® line of products are volumizing gels that are typically placed either superficially in lines or folds or deeper for after facial volume loss has occurred. Semi-permanent fillers such as Sculptra® can be placed deeper in the tissues to provide long-lasting volume correction where it's needed most. As an added benefit, Sculptra® stimulates your own tissues to produce collagen and slows the progression of aging. Consequently, Sculptra® is considered a unique class of facial fillers, known as bio-stimulators.

What types of fillers are available?

How long will the results last?

Dermal fillers usually last 6 to 24 months or longer. Fillers are improving all the time and many last for 2 years or more! How many treatments will I need?

Dr. Miller believes that dermal filler treatments offer the best results when performed regularly, with treatment sessions typically spaced several months apart. Individuals interested in Sculptra will require more frequent treatment sessions initially, usually every 6 to 8 weeks. In some cases, dermal fillers may be combined with BOTOX® Cosmetic and other skin treatments for optimal results.

Which product is right for me?

We will evaluate your skin during your personal consultation. During this time, an individualized treatment plan will be created to give you the best results based on your goals. People with significant skin sagging or excessive skin laxity may be better suited for facial aesthetic surgery.

Step-by-Step Procedure Facts

  • Your procedure will be performed at our Aliso Viejo office.
  • No anesthesia or pain medication is typically necessary prior to treatment.
  • To reduce the risks of bruising avoid aspirin, all herbal supplements (including fish oils), and products in the NSAID class of medications: ibuprofen, Advil, Motrin, naproxen, Aleve, etc.
  • You can return to most of your normal routines immediately.
  • Some minor swelling, redness, and bruising may appear initially at the injection site.
  • Improvements will be visible immediately with most fillers, but will continue to improve over time.

What is a "liquid facelift?"

"Liquid facelift" is a popular marketing term used for a treatment that combines a wrinkle reducing injectable (BOTOX, XEOMIN) and dermal fillers to create a comprehensive facial rejuvenation. Here at Refreshed Aesthetic Surgery, we call this treatment a non-surgical facelift because we sometimes incorporate an energy treatment such as laser resurfacing. This combination of products and treatments, performed by Dr. Miller, can take years off of your appearance. However, it is important to note that for patients with a greater degree of skin laxity and/or volume loss, non-surgical treatments may not produce the significant and lasting results achieved with surgical procedures.
